How does the single player difficulty work? I was under the impression that after beating the game, or after reaching a certain level, and restarting the game, the difficulty would be increased accordingly. Otherwise replaying the lower levels would always be a total waster of time after around level 15. I have read some contradictions to this assumption and some say that single play difficulty is always normal and that you have to use a multilayer character to access higher difficulties.

This is indeed the case.
