At the end of Heaven's Feel, which is a reference to anal sex, he ironically comes to terms with his sexuality in a frank talk with Kotomine, a sociopath priest who had been trying to molest him throughout the story. Shirou could sense this about him but had not put the pieces together until the very end. While he didn't want to be lured in by the priest he also couldn't help but feel a kinship with him as the only other homosexual in the story. He even falls for him in a certain sense but cannot accept his advances.

The fistfight is a metaphor for Shirou defending himself from the priest's final, desperate attempt to have his with him. The "heart of darkness" allusion is so obvious that it doesn't need to be explained, but "Angra Manyu" is either HIV or cancer. Either way the priest doesn't have long left and decides to force himself on the object of his lusts.

It's a very tragic story, really. A love that could never be.
