Mine is Doom 3, it may seem strange since you play as a space marine that carries a huge arsenal of weapons but I was constantly on edge while playing that game due to the creepy atmosphere and the fact that enemies can just jump out at you from nowhere (can't wait for the BFG Edition).
Also I've never really found the Resident Evil games to be very scary myself, so I find it strange that one of the main complaints about the newer games is that they aren't as scary as the older ones.

Silent Hill 1 and 2. Dead Space 1. BioShock believe it or not (first one). System Shock. Siren.

Vampire Rain and Hunted The Demon's Forge... But that's more of a "How did this get past testing" kinda thing.
As for creepy...
Silent Hill 1 - 3 were good. Never got to play Siren...Dead Space... And I think that would be the only ones that were any bit scary.
