Are they a random occurance?? Or can I purposely perform them somehow...?

I dont think there is any way to perform them on command.

I one shotted some bandit bint with my axe though. Epic.

Get them down to about 1/4 health then hold RT and you'll do it - I have about a 50/50 chance when doing that.

It happens ot me all the time.  Sometimes I hold but most the time I just slash them like 3-4 times in quick consession and it cut scenes to me screwing them up.  Or if they are getting up or drawing their weapons who can sometimes get them quick.

It seems to be related to over-damage, meaning, if you hit them a lot harder than the amount of health they have remaining. That, or it's some sort of random critical hit type thing.

Seems to be random. The 1h mace/axe animations are broken when attacking most creatures though. At least they are for me.
