Unarmed combat gets a damage bonus from 2handed.
In heavy armor there is a perk that increases your damage by the armor value of the gauntlets you are wearing...
On top of that, you could build off of the heavy tree to get multiple increases in armor value, which in turn increases you hand-to-hand damage.

Khajit Unarmed Bonus (+15 damage)
Unarmed attacks with heavy armor gauntletsódamage increased by gauntlets' armor rating
Increase armor rating by 20% (5 ranks)
Additional 25% armor if in matching set
25% armor bonus if all in heavy armor (not matching)
2H weapons do 20% more damage (5 ranks)
Standing power attacks do 25% bonus damage, chance to decapitate
Sprinting power attacks do double (critical) damage
Sideways power attacks hit all targets
Backward power attacks have 25% chance of paralysis

All of these things could go into the lethality of unarmed.

There are enchantments that boost hand-to-hand damage as well.

I'd love to see them decapitate someone in game with their bare hands.

A shame you have to be wearing heavy armor to do this. I was hoping for a monk with little to no armor.

The only problem I see is a monk type (light/no armor, agile) hand-to-hand character doesn't seem likely unless there is a unarmed tree that hasn't been mentioned yet.

You basically have to aim for a walking Juggernaut that punches people to death.

You could just stick with the gauntlets. The damage bonus comes from them only.

This is true, and it's only going to cost you about a 50% damage increase, so it's not terrible.

Aye, but the pre-req for the perk is a high skill, so you better hope you get hit in the arms alot.

I thought the gauntlets perk was like the second one you can get in Heavy Armor..?

Anyway, this may be the first time I do an unarmed character. If it's not in the main game, I'm sure it's just a matter of time before someone makes a mod that lets you punch heads off.
