How many playthroughs do I need to get earn the Platinum?

If only one playthrough, do I need to play on the hardest difficulty and what else to do to make sure I earn all the Trophies in one run?

It's required to do it in 2 playthroughs. By the middle of the second playthrough, you should get fully powered up. I remember it's usually consistently in one area of the Green Zone where you fight 3 Evolved who were all posing as humans in one of those "scientist studies" sites. One was a scientist, the other 2 were Blackwatch soldiers.
