long press the Vita screen until the menu pops up. Should be the first option in multidisk games.

IIRC, wasn't there a problem with MGS and switching disks on the PSP? What about the Vita?

Or am I just not thinking right?

If you need to change the disc when you first load it up, just hold the front screen till the menu pops up, touch the Reset Game option and then it lets you select which disc to load up.
