I've never really seen how the "Squall is dead" theory could possibly be true. The ice spear is clearly just going through his shoulder in the FMV, and even if it had hit an artery the ice itself would stop/slow the bleeding long enough for healing spells to be cast on him. And he would get healed, if only so he would be able to survive the inevitable interrogation about who had hired SeeD to assassinate the Sorceress.
