There is a voucher code on the back of the manual that you have to use.

After a mission with General Washington , its between sequence 8-10 i believe. Not sure when it opened just noticed the icon.

Either Sequence 9 or 10. Can't remember much the exact number at which popped up, but it was close to the end.
