So what exactly was the "Dream Team" back in the 90s on the N64?

Ive heard a little bit about. I know it was a like a group of developers and publishers made to work on the N64. But here are my questions:

1. What the Dream Team's purpose? Why did it exist? What was its goal?

2. Did it accomplish its goal. What was the end result of the Dream Team's existence. Has Nintendo ever tried to assemble a new dream team?

3. Who exactly was in the Dream Team?

4. Where did the name come from?

5. Who's decision was it to make the dream team? Nintendo's CEO?
