why is this game rated M?

Because its a Video Game.

Rating system is biased, since most still believe video games are just for kids.

Everything you see in this game can be found in a lot of PG13 and some PG movies.

Sure, but Arioch's design can easily be mistaken for just an ugly second mouth. I didn't recognize it for what it really was until someone pointed it out to me.

On the other hand, NO ONE can mistake Mara as anything other than a giant green dick on a chariot.

I'm pretty sure that there are a few personas with visible nipples too, or at least one (who's name I forget).as well

There are a few naked ladies, but I think they're all covered. Or edited... Alraune, for example, has had a very convenient rose added.

I was only gonna count female nipples, since male nipples are shown in T rated games and PG movies/TV shows all the time...female nipples on the other hand...
