Haven't played Bioshock so I don't know, but that image doesn't seem to match up with anything I find on google about her... maybe it's someone else?

This cosplayer has a similar Little Sister outfit as the one in the image I think.

All we know is that her pixels are small and she is besides Big Daddy. Clank is besides Ratchet.

Chances of it being a Little Sister (i would say) 90%

Never saw Little Sister on the Bioshock 1 cover... Huh. I guess that is her then.

Seems like the image is really lazy, taking copy+pastes from game boxarts. We see Jak 3, Ratchet Crack in Time, inFamous, etc.
