It's supposed to be random. There's a bigger chance if there's 4 players I think.

it comes out random when you press up circle. it only comes out offline as well, as it is just an easteregg.

Never was, never worked online. I have been begging for an answer of this as well. --- If you have a mediocre connection, don't bother playing online.

Im pretty sure you have to have items turned on also for it to spawn. --- If you can't come in from the cold, then you gotta grow ice over your heart.
