They use his Sly 4 design.

Basically a combination of his outfit from 1,2 and 3 and he looks a bit older.

They gave him the backpack from Sly 1 and the bag around his leg from Sly 2 and 3 so he has both.

I found out by me quoting you in my first statement, my answer turned into a misleading one. Read it now.

Also in addition Sly's base appearance is still the same.
