Wrong. Hold L2/LT to throw the grenade, and while he's prepping, hold the shoot button. You are now holding the grenade until you let go of the grenade button

There's a difference between cooking a grenade and holding it with the pin removed.

You can do both.

It's not recommended, but you can pull the pin and hold it or pull the pin and "cook" it, so I don't see how that's not realistic. Especially because pulling the pin and holding is more likely to happen in reality.
