There are a few changes to the normal DD game there is now a port crystal just outside your home town, you can select to play the game on easy, normal or hard plus port crystals are easier to find and ferry stones are cheaper.

There is a new isle called bitterblack isle in DD:DA which is full of high level monsters which you haven't seen and lots of new gear to find.

All in all if you enjoyed DD and are looking to maybe get back into playing then DD:DA will pull you back into the world.

Just to worn you bitterblack isle is pretty tough best to have a high level before you head over (80-100) would be a good start.

Are the new bosses only in the isle or is it also in normal DD?

There are some few quests where new monsters appears on normal DD areas, but they are exclusive of the isle.
