NAT issues - Please help.

I'm having issues with my NAT.
I have a combination Modem/Router, but there is no other router involved anywhere. I have UPnP enabled.

My problem is that my Xbox One lists my NAT as "Open," but Call of Duty Ghosts is listing my NAT as "Moderate."

I would suggest getting a separate modem, and give the modem/router back to your ISP. And buy your own router too.

You'll probably get a credit of $5 or so per month (even though they don't itemize it, you're still getting charged for their equipment).

Go to portforward.com
Find your router
Follow the steps to open the ports

Its very simple and step by step.

See if you can open the DMZ of your Xbox's IP and make sure its static, fixed my ever changing NAT.

This also isn't addressing why my Xbox One says my NAT is "open" and games (game) says it's Moderate.

Is it possible that the Call of Duty servers demand different ports than the Xbox One / Xbox Live?

Dunno. Perhaps.
I don't have ghosts on xbone.
Bought it on ps4 first.

No, the test in the dashboard is a general guideline, it's not an exact answer that your network is set up correctly. It might be good enough for the test, but the real test is connecting with your friends and finding all the matches, etc.

Then I would suggest getting a separate modem and router. Combos are bad.
