Gonna say Dugtrio because it's just three Diglett with angry eyebrows and they kinda reused the "one-to-three" thing with Magnemite/ton... though the latter works well because magnets. It was also used to a lesser extent with Koffing, to boot.

I could argue that many pokemon aren't 'imaginative,' yet that number is stunningly high.
Two do genuinely stand out though.
Voltorb and Electrode. They're supposed to appear as pokeballs.
A good idea, had the pokeball not been part of Pokemon's series. They implemented a self-reference in the first generation.
The concept that they look like pokeballs and trainers confuse them as pokeballs is also flawed, because both Electrode and Voltorb are gargantuan compared to a pokeball. Electrode especially... being 3 feet tall and 3 feet wide. >>;
These two pokemon are not only uncreative; but terribly executed. :/
