Ok, I've played pokemon for forever but this is the first time people have started using terms like "5 IV" and other stuff. What makes pokemon perfect and what does "5 IV" mean!? Thanks!

a perfect pokemon has 6 maxed IVs and 5IV means it has 5 over 6 IVs which is maxed.

at level 1, if it shows 6 in all the stats and 12 in HP is that perfect? because i have a level 1 larvitar that has that but it has a neutral nature.

Not necessarily. Only way to tell for sure is to take it to the IV judge.

when people specify 5IV pokemon. does it usually mean 5IV in the correct stats? like haxorus' 31/31/31/x/31/31 or just any combination of 5 IVs?

People often specify whether IVs are perfect or imperfect.
Perfect = 5IV in the right Spread
Imperfect = 5IV without right Spread
Flawless = 5IVs in right spread with 0 in the non desired stat

From what I understand pokemon have 3 modifiers along with their level value to determine the range of their stats.

IV: These values range from 0 to 31 in every stat and cannot be changed once the pokemon is obtained. They are random when caught in the wild, but can somewhat be influenced when breeding to get a better outcome.
EV: This is stat EXP, depending on what pokemon you train on, your pokemon will obtain different stat exp assign to their stats to increase its value. They range from 0 to 252 in every stat, but have a max of 510 overall.
Base stats: Values unique to every species of pokemon, are never changed.

The game puts these 3 values and your pokemon's level into an equation to give its stats. (Well something like that, don't quote me)

When people say 3 IV pokemon, they are refering to pokemon with 31 (max) values on atleast 3 of their IV stats.
