Empty Grand Soul Gem Locations
At higher levels, and with a high Luck attribute, Grand Soul Gems begin to appear as random dungeon/cavern/tomb loot. They can also be found in the following locations:

Zebabi:
Look around the area where Draryn Andrano is, and you'll see a small opening in the wall above him. You'll need Levitate or Jump to get in AND out.

Vivec, Telvanni Vault:
There are trapdoors leading to the actual vault, one of which will put you in a room with two Storm Atronachs if you pick the wrong one.

Vivec, Redoran Trader:
The same louse who sends you on a quest to haunt another trader's shop has this soul gem in his back room.

Tower of Tel Fyr, Hall of Fyr:
A pair of them are on a shelf on the lower level, in the room with the large bed.

Tel Vos, Aryon's Chambers:
This one is on a ledge behind him, hidden by a few giant roots. You'll need Levitate or Jump to get it.

For all three of these houses, you will probably need to have the stronghold of Tel Uvirith at its Level 3 stage - otherwise, one or two of these "houses" may not appear.

Tel Mora, Tower Services:
You'd think that there's only one of these soul gems in the room with Salam Andrethi (on the shelf that's behind the large jars), but wait - there are two more hidden near the ceiling. Facing the entrance to Andrethi's room, stand on top of her bed and Levitate, heading towards the ceiling. Look forward again, and turn slightly to the left. You should see the other two soul gems on a small ledge. If you can reach the gems while standing on top of the root nearest to the ceiling, then you should be able to Sneak while attempting to grab them.

Salmantu, Shrine:
If you choose to enter this area, then proceed with extreme caution, for it happens to be a lesser base of the Sixth House Cult.

Empty Grand Soul Gem Locations, Part II

Maelkashishi, Shrine:
It's fairly easy to miss this one, as it's NOT in plain sight. Enter the shrine and stand right in-between the feet of the Sheogorath statue, and then turn until you're facing in the same direction as the statue. The soul gem is on top of the pillar right in front of the statue to the left. And just so you know, the other Grand Soul Gem in this room (on top of the small chest behind the statue) already has a soul in it.

Hanud:
Another hard-to-find one, if you don't know what you're looking for. In a small room inside this Telvanni tower, you'll find it along with a random tomb enemy, a skeleton sitting against a wall next to some barrels and a wooden staff, and a fire pit in the back of the room. The soul gem is hidden behind the barrels.

Dun-Ahhe:
This one's even worse - it's in an area of the cavern near a set of strange crystals growing out of the ground. You'll find the soul gem near a skeleton sitting atop a rock with several enchanted arrows, a long bow, and one Glass Arrow.

Derelict Shipwreck, Cabin:
You'll probably pass by this ship run aground when you're on your way from Khuul to pay a visit to the Urshilaku Camp during the Main Quest.

Bal Fell, Inner Shrine:
If you're after the Ten Pace Boots, this one's hard to miss. It's on top of the chest that the boots are in, right in front of the Sheogorath statue.

Ald Redaynia, Tower:
This one is sitting on an altar with a Greater Soul Gem and the Bittercup. Of course, the only thing standing in your way of getting it is a Level 20 Skeleton War-Wizard with the Vampiric Ring.

Dren Plantation, Dren's Villa:
You'd better be careful when going after this one. It's in the lower room of the main villa inside of a locked chest (the one in the screenshot) - along with a ruby, a diamond, and a note that can be used to blackmail Orvas Dren. Also in this room are the Ienith Brothers - attack or kill one, and the other one appears right behind you.

Venim Ancestral Tomb:
The last stop in a very obscure Misc. Quest, this soul gem is being held by a necromancer named Goris the Maggot King. Accompanying him is Luven, a man who betrayed his bandit friends to Goris. Of course, you'll probably have already killed both of them while attempting to claim the Bow of Shadows, which is in the same room as these two.

Aside from the mention of the other one in Maelkashishi, this guide does NOT direct you to any of the Grand Soul Gems that already have souls inside of them, as any Grand Soul Gem containing a soul that is not a Golden Saint or an Ascended Sleeper isn't worth looking for, much less taking.
