1. Space Paranoids, in a chest at the computer core
2. completing Atlantica
3. completing 100 Acre Wood
4. chest in TWTNW, Brink of Despair
5. chest in Twilight Town, Sunset Terrace
6. complete Goddess of Fate cup
7. collect at least 1 of every other synthesis item, including one other Orichalcum+ (maybe you just forgot to give your items to the Moogle? That's a commonly found mistake. You have to give the O+ he gives you back to him as well)
