The ceiling for the sliders in GTA IV are ridiculously high.

What's your system, I doubt you're running Skyrim @ Ultra settings, but GTA IV at medium --- i7-920 @ 3.6 // 470 GTX // 12 GB G.Skill Sniper Ram // PS3 // 360 FiiO e9+17 // AD700 + M50 // Deck Legend + 82 // DAS Ultimate S

In my experience, it's mostly shadows and viewing distances that cause the problem. Don't set shadows to the max setting, turn viewing distances down a bit, reduce traffic density (it'll be less of a PITA to play that way, too) and you should get much better performance. --- -What is best in life? -To crush your enemies, see them driven before you, and to hear the lamentation of the women.
If you're only running GTA IV @ medium settings w/ that system something is terribly wrong. My system can run GTA IV close to max.
Also, I can't change ANYTHING. It doesn't let me change the settings at all for GTA
