I seem to always have to do it, otherwise it tears.

I only enable v-sync when the tearing is really bad.

Yes because I'd rather have 60 fps and no screen tearing.

Only if there is tearing and no option to limit the framerate. V-sync causes so much input lag it's almost unbearable in some games, a much better option is to just limit your framerate, but for some odd reason most games don't include this option.

Pretty much this... if the tearing is very minor I ignore it... if it's terrible, I enable it (or in some cases I can just change the max_fps value to minimize tearing)

Everything else - yes, unless the input lag is atrocious.

Only time I enable it is if I'm playing an older game which I get hundreds of wasted frames per second.

Usually.

Some games the tearing is minimal enough that I only notice when I'm not focused on the action.

Yep. Screen tearing usually makes me feel sick after a while.
