Usually it's the specs needed to run medium at 30 fps.

There is no industry standard for performance, it's just what the developer recommends.

it's vary a lot from a game to another , but usually Recommended get the game to work at a high resolution with no lag at all.

Nah recomended is definitely not high more like medium. My current pc is just above the recommended specs of some of the games that i play and can not run on high without getting like 10fps (im planning on building a new pc shortly but thats a whole other discussion).

They nearly always make it so that the high or ultra settings (if there is an ultra setting) is way beyond what computers can do at the time they release the game. That way they extend the life of the game a bit longer since graphics are always getting better every 6-18 months.

I'm sure recommended is what they planned for the game to look like at the time it was released....which is to say medium or medium high settings.
