The most impressive mod I've ever played was Alien Swarm for Unreal Tournament 2004 I think. It's too bad the free Steam version didn't get more attention.

Freespace 2. Some of the stuff put out by that community is seriously better than a lot of triple A titles these days.

Also, I did expect quite a lot of Bethesda game answers, but there are more than I expected. Tried modding Skyrim once, but I couldn't find anything particularly interesting that didn't clash with the original game horribly.
