I pivot my arm from my elbow for long horizontal movements
I pivot my hand from my wrist for small horizontal movements

I protract/retract my fingers for small vertical movements
I shift my whole arm up for long vertical movements
though sometimes it's a combination of both for vertical movements

My palm doesn't make full contact with the mouse, it's mostly just the ball joints below where the fingers start

I play 30cm/360 turn (11.8in/360) with a Zowie EC2

I keep my wrist basically stationary and pivot from elbow, or from shoulder if it's a large motion. For small vertical changes I extend/retract my fingers.

Mostly wrist positioning with my palm planted for course movements and fine tuning using finger movements. I try to set mouse sensitivity for anything first-person perspective so that a full horizontal movement of my wrist without repositioning my palm results in 180 degree rotation.

that's a great rule-of-thumb for sensitivity
I went for the lowest sensitivity I could have while being able to comfortably do a bit more than a 180, and the result was 33cm/360. I raised it to 30 a few weeks later though (It was a bit hard to move far enough with my wrist-pivot movements while sniping in TF2)

This is me exactly. I play on a Wheel Mouse Optical and the sensor positioning is perfect for wrist pivots.

Also, I've settled on a sensitivity standard in which the distance that the mouse is moved reflects the distance moved by the mouse on the screen for FPSs. This reduces a lot of my stress when starting a new FPS and am trying to find the optimal sensitivity.

Honestly, pretty much this

I use my ring finger/pinky and thumb to grab the sides of the mouse and move it.
