The mini hdmi most likely caused an electrical short. The metal ground for the hdmi plug most likely made contact with the +5v +data -data -ground, sending the +5v to the data paths frying the controller.

Twice? What are the chances?

Is there any troubleshooting I didn't do that could have been the issue? I guess I'm just wondering if I'm jumping to the conclusion that the HDMI plug destroyed those ports, but I'm out of ideas.

The chances are easily high. This is almost exactly like sticking a flat head screwdriver into the usb port, it would make contact with the prongs. The prongs in the female port are raised so it would be really easy to make contact.

The only trouble shooting you could do is to visible check each usb port and ensure none of the prongs are making contact with each other or the ground shielding and enable/disable them in bios.

The usb device accepts generic drivers and are installed automatically when booting the OS(assuming your using any windows version beyond 97). Windows would most likely either be not reporting that usb host or reporting a device malfunction. So you would know on boot if the keyboard/mouse works.
