Volgarr the Viking. Just came out and it's amazing. Very challenging. It's more akin to 8bit/16bit Castlevania or Ghosts n Goblins.

I think it's 9.99 on steam.

This. Volgarr the Viking is pretty solid.

La Mulana, which is on sale for $3.00 (80% off) on Steam right now. It is fantastic if you can get used to the controls (and it is very difficult.)

Spelunky is very challenging, as well - it is procedurally generated, if you're into randomness.
Miroku2235: Every time someone says the Mesmer reminds them of Naruto I die a little inside.
Groadergreen: I'm legally deceased because of things like this.

Check out They Bleed Pixels. It's about as difficult as Super Meat Boy, especially in the later stages, with an H.P. Lovecraft influenced art style. It's a mix of challenging platforming and unique one-button combat that is very underplayed imo, but also very polished with a lot of level achievements/goals to reach beyond just beating each level. Controls are also as tight as Super Meat Boy, which is about as good as it gets with platforming precision.

This. It's really tough. Love the game.
