recently they'll stop working and i'll have to reboot my PC for them to work. Is this a sign of some kind of power failure?

Possibly, check your cables and the ports they are plugged into on the motherboard and see if you notice anything unusual.

I'll check when I get home. When I plug something into them, the power light on the device i'm trying to power will flash quickly. When I reboot my PC works perfectly. had this PC for about 6 months or long and it's never done this.

Say if the ports are bad, is there a way to replace them on the case?

Check the internal connection to the motherboard. Try changing it to another controller if your board has one.

check drivers and power saving features as well.

Device Manager, Universal Serial Bus Controllers, USB ROOT HUB, Right click/Properties, Power Management, uncheck "allow computer to turn off this device to save power"
