They're console games, but the Ratchet & Clank games always come to mind when I think of New Game + done right.

You started with all your old weapons and armor, and you can upgrade your weapons another round and there's a feature that adds multipliers to the amount of money dropped based on how many enemies you kill without taking a hit.

Recently, Borderlands 2. It has new enemies, new weapons, new... well, pretty much everything except level design.

To be fair, its only possible because of their loot system, which randomly generates parts based off individual pieces/levels/rarity

Another idea I had was the spy/sniper being able to climb into the soldier's rocket launcher and be launched across the map, dealing more damage, but no splash.
