Just curious about the state of ssd's. Please list the type, size, and how long you owned the ssd before it died if you had one die on you. Also if you have had good luck with one list that also if you like.

Samsung 830 x 2 at 256 GB each in a RAID 0 array. They're still fine after over a year

A 128 and 256 Crucial M4 for a year, no problems.

Most people are recent SSD adopters (if they have one at all) so I doubt there will be a high reported failure rate.

Bought my first SSD a 128gb Sandisk Ultra for my laptop in August of last year. It crapped out on me around February. RMA'd it, and Sandisk sent me a DOA SSD. Called them and got a brand new one. Working fine now, however, I barely use my laptop.
It only had a 1yr warranty, and this was way before I really knew much about computers. Never buying another Sandisk SSD ever again though. I like the 5 year warranty on my Intel 520.

Only got my first SSD, A VGen OEM, which is actually a Samsung 830 inside, 128GB.

Same SSDs, same configuration, same time, still fine. Also I have a 960 GB Crucial M5 that's been going fine for about 6 months now. Never had an SSD fail on me.
