A 4k monitor and DisplayPort/HDMI 2.0 if you want higher than 30fps.

4k is really difficult to achieve right now. The monitors are extremely expensive, your card needs to support 4k (i could be wrong, but I think only AMDs newest cards support it) and if you're using HDMI, you need HDMI 2.0, and I don't think a single video card at the moment supports it. Just wait a few years. I myself am waiting for 4k OLED tvs to be reasonably priced, and I know i might have to wait 5 years for that.

Not too far off, if not completely accurate.

The issues with gaming at 4K resolutions are obvious: the 38402160 resolution requires the same horsepower as four standard 1080P panels combined.

I'm not sure there's even a solution out there that will give you 60FPS at all points with resolution like that.
