Probably GTX 660ti, or add about another $50 for GTX 760.

I'm running a i5 2500k @ 4.3ghz + Evo 212+ Cooler
Ram : 16GB 1600mhz Mushkin
SSD SATA3 drive


1920x1080

My video card was always the weak spot. Now that it's dying, I'd like something that will last for a long time in 1080p and still pull the Ultra settings on 2013 and future games.

You definitely want a 760 then, I suggest saving up if you haven't got the money yet.
Everything else in your PC, except maybe your hard drive, are overkill already.

As everyone has already stated, a 760 is your route. 200 isn't really a sweet spot for price for performance ratio. 250-300 and you have a lot more options to play with.
