The Secret Island of Doctor Quandary. Was maybe 5 or 6 when it came out, loved that game... Still remember most of the puzzles!

Well, depending on what size floppy we are talking about is is either Tetris or Word Rescue (possibly Where in the World is Carmen Sandiego).
