I don't think it would change too much simply because Japanese-only releases (especially for an ancient console) weren't played much by American audiences. Also, you only vote on 10 games...most ppl's top ten will probably include SMB3, a Mega Man game or 2, Zelda, and Contra...that's already half their list or more.
Put it this way, Akumajou Special: Boku Dracula Kun, is one of the better platformers I've played on the NES...but how many people have heard of that? I've heard Lagrange Point is one of the best RPG's on the console...but nobody has fully translated it...so the people who would include that on their list not only have had to have heard of it...but they also have to speak Japanese to understand and play the game. Earthbound is probably the only Japanese game that would be sure to make the list...other than that I'm not sure if anything would change (a lot of people don't like FFII & FFIII...and I haven't heard anybody talk about Wai Wai World or the Fire Emblem games). Even Sweet Home...I'd say its a good play...but even me (being an RPG fanatic) wouldn't put it in my top 10 vote for the list...there are better games and better RPG's out there to include. --- A computer once beat me at chess, but it was no match for me at kick boxing.
