This game is called "Secret Command" in America?

I guess Sega just couldn't get the Rambo license or even try to. Oh well, its still the same game.

the original cart was Rambo, not sure why its called this now.
There was 2 Rambo games on the SMS in the US, this one and a light gun game.

So yeah, Sega did get the license.

Yes, they did get the license originally but they couldn't get the license or didn't even bother to try get the license for the Virtual Console re-release.

Which is why they released the European version of the game in America.

Actually this game was called Ashura (阿修羅) in Japan. However since the game featured so many themes inspired from various Rambo films that it was renamed Rambo: First Blood part II in North America. However in Europe and Australia it was given the name secret command.
