Mutant League Football. One of the best games ever made. It was perfect besides the fact they stopped making them.
Games that are close:
Military Madness: Loved this game for years. Multiplayer usually sucks though cause it uses the singple player maps so usually green is OP for a real person to use.
Fire Pro Wrestling on saturn. Had to import and print out guides to translate, would have loved a english version.
Mass Effect 2: could use more guns, a few more powers, and now after ME3, no multiplayer.

Star Ocean 3 was right up in there. Forever a top for me. I have a bunch of games that have been near perfect, SO3 bein' the best. Tales of Vesperia, Persona 3 were up in there too. I'm just waiting for the one thing that will cease my undying love for SO3.

That game made me experience what it was like to be in a giant robot anime.

R-Type Delta is my perfect game. The ships, the difficulty, the challenge - all of it combined for a perfect degree of fairness, and it was always fun. Absolutely perfect. No other 2D shoot em up reached this level until I played Under Defeat HD in 2012... and funnily enough, that's an HD port of a seven year old Dreamcast game. --- Your name is sung and tattooed now on my heart - here I will carry you forever CarryTori Amos

There are very few things that I would change about these games. Out of these, the one that's closest to perfection for me is TTYD.
