Iron sights
A much more choice based story, in that you can pick any of three factions to align yourself with (it's pretty linear up until you get to New Vegas, but then it really opens up)
A better story and characters (though I liked Fallout 3s story and characters too)
About twice as many weapons
Way more enemy types
A weapon modding feature
The return of the reputation system in addition to the morality system that was already there A way better way to control your followers
Harcore mode
Melee special attacks
A new world to explore
Way more Vaults than 3

Thats what I can think of off he top of my head.

Fallout 3 was great for its time. New Vegas, however, outclasses in nearly every way, and is one of the most intelligently written games of this generation.

3 had followers as well. NV gave them storylines and personality.
