I've replayed FFXIII 3 times since it came out. I've finished all of the Final Fantasys including 11 and it's one of my favorites in the series.

Banjo-Tooie for N64 is definitely the game I've played through the most times. Close second would be Donkey Kong 64. Guess I was a big Rare fan back in the day.

As for PS3, I believe Metal Gear Solid 4 takes that spot. Seems to be a summer ritual at this point to play MGS4 once through, cutscenes and all. Always discover little parts of the game each time I go through it.
