Always normal on my first play through unless it's a series I'm familiar with and can handle pretty well. I started out Uncharted 2 on hard.

Normal. I guess that'd be medium.
3 things I dislike before a game release:
The wait, the haters and the trolls

Usually on normal, unless there's a new game+/carry-over system then I sometimes start on easy.

It depends what game it is. If it's a game series I'm familar with then I'll put it on the hardest one. If I'm new then I'll go with normal and adjust if needed. Some of the best playthroughs IMO are on the hardest difficulties.

depends on the game if it's hard or not, generally medium or whatever the game starts out at. Only time I'll consider it is if it's trophy related like Uncharted.

I always do my first playthrough on Normal and if I like the game enough I crank it to max for any repeat playthroughs.

Depends on the game. Most games Normal though

I try for hard, as I prefer to play the games with some grey matter, but I was playing Halo 4 on Heroic the other day, and it was pissing me off fiercely because for some reason my needler didn't home like the needlers that almost every other enemy was wielding.

Unfortunately, I perfer Heroic because the Elites cloak, but on lower difficulties, it doesn't even feel the same, on normal. because it's such a huge drop in challenge.and i'm not bored with it.

So yeah, I usually stick to hard

For first playthrough, always Easy. Story is my most important factor in a game.
