Always the hardest difficulty available. I'm not a slouch gamer and I usually can take any challenge thrown at me. The only games I don't start on the hardest difficulty are hack and slash games (DMC, Bayonetta, NG) because they take alot of skill to become proficent. If it's a shooting game, hardest difficulty always. It's mainly so i can get the achievements/trophies anyway and It offers me longevity when I play the game for the first time.

Always the hardest difficulty available; there's usually not that significant a difference between modes anyways these days. xP

I put everything on hardest possible first playthrough. It really requires you to think and use everything you have, and everything the game offers. I got Dishonored yesterday and I got ZERO patience for stealth, yet I put it on Very Hard immediatly so I won't slash my way through the game with ease and end up being bored.
Perfect example would be Kingdom Hearts 2, if you put it on normal you never have need for drives/limits/potions/cure etc.
Only game that proved impossible to me is Ninja Gaiden 2 on MNM >_<'
