50% ADD- situational awareness slipping while kiting and ending up trapped in a corner or something to that effect.
20% elites/champs with affixes that give me an extreme disadvantage
20% Personal Error- I've screwed up, I know I've screwed up, I know specifically HOW I've screwed up, I know I'm going to die in a couple seconds, and I can't do anything about it now.
9% lag at critical moment.
1% "WTF? How did I just die?" (This category actually covers about 20% of cases, but 19 of the 20 overlap with the first line).
