It probably doesn't get rid of blood in the cutscenes; even with the option on, blood stays in the cutscenes of MGS and other games I have. Anyway, that would require them to create the scenes twice, and I could see that as being a waste of time and money for the creators.

I know there's an option to turn off some sort of blood effect, not exactly sure what it turns off though since I've always left it on. There's not much blood when killing things. It's mainly just the blood splatter on your body that might get annoying at times and maybe that's what the option turns off.

that splatter IS annoying, partly due to the lack of realism... "whee! I just killed a rat! Now I'm covered with a pattern of little blood splatters, head to toe. Thankfully I have a dog who can lick it all off. yyyyaaaaaay!"
