what is out of combat exactly? 3 seconds after an attack? 5?? 10????

For that I usually look to graves' passive, but I can't remember the actual time

BoM are 5 seconds. Graves' passive is 3. I'd guess it's 5 seconds.

5 seconds after not taking damage (ie towers, minions). Debuffs count as well.

To be fair, a lot of champions never really had "unique" passives, like FoN and Mundo's passive, Veigar and Chalice; at least, they're very similar to some items.
