Flask + wards usually. Rush tear (and asap Manamune/Muramana), then get items like:

Frozen Heart
Spirit Visage
Iceborn Gauntlet
Warmog's
Runic Bulwark
Randuin's Omen

start pots/faerie/ward or flask/pots/ward or pots/ward

get tear
rush defensive stats to counter lane, ex glacial against ad, dragonball->SV
after you have completed one fo these get giants belt
finish other defensive stat to help against the overall enemy team
compensate more defense if needed
now get your manamune, since it should be at the 750 mark now
finish your warmogs
If you decided against FH you can upgrade your glacial into a cold hand
you can get TF if you feel you have the money and you can absolutely tank everything
more defense
