That's how it *should* work, and that's usually how it works for competitive play or team 5v5s. But you can't expect this to ever happen in solo queue. Like, at all. As long as pick order is observed, I'm happy. It's nice when you get the few players willing to hold characters for a swap or whatever, but it's so rare and hard to implment that it never happens. Which is why Riot is looking into changing how the entire pre-game works.

I'd say AD should pick before support. You have a lot more knowledge about what kind of lane you're entering when you see a Leona or a Blitzcrank or even a Soraka rather than a standard Caitlyn/Ezreal/MF pick.
