Can Items Disappear If...

I put them in a follower's inventory? I gave Boone and Rex a bunch of guns and armor before starting Honest Hearts, and they were gone when I came back.

There are numerous bugs that will make items disappear from your follower's inventories. Sometimes this will happen at casinos.

Did you dismiss your follower before accessing the DLC, or did you get the "your follower has returned to..." message when you accessed the DLC? It's always best to dismiss your followers yourself instead of having the game do it.

I've dismissed and re-acquired Boone many times and he hasn't ever lost his inventory, not sure about Rex as I never use him. Luckily, unless they were unique items/weapons, you'll eventually be able to make up for your losses. Tons of guns, ammo, and caps in this game, particularly if you have the GRA DLC.

As a general rule, always have your followers waiting outside when you enter a casino or any other place where you have to turn in your weapons.

Doesn't matter if they wait outside or not. That bug can occur regardless.

Aye, even if they are waiting, they will be stripped of their gear when you enter. Only way to avoid it, is to dismiss them.

It wasn't a casino, I think. I swear I remember having them after the last time I left any casino.

Any time your companions are "with you" and you enter an area that forces a disarmament, like casinos, or the Fort, they can lose their weapons.

Whether or not you might of gone to one of those zones before heading to Zion, that might explain it. Otherwise, no idea what to tell you, have not heard of followers losing weapons just for doing DLCs.

Not sure if they lost their stuff during HH, but I gave them to them, did HH, continued with some other stuff, decided to switch from Energy Weapons to Guns, and saw all my guns had vanished from Boone's inventory. Is there anything else that could cause them to disappear?

It is possible. But the only ways that I know of followers "losing" weapons are if they are taken by a casino, and never returned, or alternatively, having them shot out of their hands.
