Why does this happen?! It's so annoying... It's different from the attack missing, I think, and I can't stand it. e_e

I guess that that could be it... But it seems to happen to me very often, and I don't think that the other Pokemon always has it

I think certain moves are scripted to use that instead. Leech seed uses that weird format, I don't really get why...

If I'm not mistaken, non-damaging attacks use "_____ avoided the attack" whereas offensive damaging moves use "_____ missed"

Accuracy=The attack missing.

Evasiveness=Avoiding the attack.

That's always been how I look at it.
