An invisible killer!?

So, I was playing a few matches and about 4 minutes later I just saw my teammates go down with no name of the person who killed them. It would only show a blank name, the gun they were killed by, and the down player.

Example: (insert gun image here) SHADQW
As you can see, there is no name before the gun image.
Could this be some kind of lag, glitch, or hack?

Yeah, i dealed with him last night, it's really annoying, he's invisible...

There is also a hack for unlimited invisibility.

You could just have a bad connection. It's actually one of the more rare glitches.

Could also be on of the "..." named players. They don't show on the screen during the game.

They don't kill you, which is the good thing. I noticed that sometimes they kill one person and one person only.

Except for when they do and you become paranoid due to being taken out by someone you can't really defend yourself against. It's all good though, no big deal.
