My 3DS won't read whats on my sd card. like none of my software appears on the home menu, I can't get on the shop because it doesn't detect the card, I even went on system settings and checked the data management and it can detect all of my extra data, but everything else appears as questions marks. I made sure the sd card wasn't locked so I have no idea what to do.

Some general troubleshooting:

-Try the SD card on your computer
-Backup/format SD Card
-Try a different SD Card

I've tried all of those....what data do I lose exactly? I know swapnotes, anything else?

Well if you backup your card before formatting, you shouldn't lose anything.

If you do it without backing up, um..swapnote, find mii / streetpass data...any DLC or extra content...and photos/music.
Probably missing some but that's the general gist

I backed up the data, formatted the card, and restored the data but the same thing happens...
